"It's ok, she shares"....

You are the midwife to a young mother who is currently pregnant with her second child. She is a repeat client, and you had to refer her to drug and alcohol treatment services last time as she was smoking marijuana. This is the fourth time you have seen her this pregnancy, and have arranged to met her at her mothers, as she often does not attend clinic. She is currently around 32 weeks pregnant and is not doing the diagnostic tests you require. On arrival, she is sitting in the garage of her mother house with around five friends. The others are drinking a premix, which is about 5 %. You take her inside and commence the visit with her. All appears well, fetal growth is good and maternal health stable, apart from her looking extremely tired. You start asking how she is looking after herself, when her mother states that she is still drinking alcohol, and would that be why she is to tired? You ask how much is she drinking, and the mother replies that she buys a six pack everyday, but that she doesnt drink all of it as she shares it with her friends. The patient then states that she smoked marijuana with the last and they were alright, and alcohol isnt a drug, so it should be ok. She then states that she is worried that if she doesnt buy the alcohol, her friends wont visit, and she will have no one to support her when she has the baby. You ask about the other baby, and she tells you that it is currently living with an aunt as the aunt doent like the older child to be around her friends. The aunt gained custody through CYPFS, and your client is very worried about loosing this child the same way. Your client begins to get upset and states that if you refer her to the drug and alcohol service, she will drop you as the LMC and wont do any of the test or see a Midwife at all. Do you refer the client to the drug and alcohol service or do you try to educate her on the dangers of drinking in pregnancy yourself, and provide the care and monitoring?
It is proposed that the client should be provided with care and monitoring.
