Paramedic Blunder - do I tell?

You work as a paramedic on a busy metroplitain station. A co-worker confides in you that he recently made a bad mistake during a cardiac arrest. He tells you that he was single crewed (no other ambulance staff in attendance) and the patient was not shocked as they presented in asystole (a flat line that cannot be shocked). He was helped with CPR by police who were also at the scene. After deciding to stop CPR, the co-worker was packing up and realised he had not connected the defibrillation pads to the defibrillator causing the 'flat line'. Had the pads been connected to the machine, the co-worker may have found a shockable rhythm and therefore, could have delivered the shock, potentially saving the patients life.
It is proposed that You should just keep quiet and forget it. He's a good company man
